OUR 5th ANNUAL GO JUMP IN THE LAKE POLAR PLUNGE will take place 3pm FEBRUARY 17th at THE THOMAS P. MORAHAN WATERFRONT PARK during the GREENWOOD LAKE WINTER CARNIVAL WEEKEND

I fully understand that this involves jumping or running into the water of Greenwood Lake under February winter weather conditions and that there are health and safety risks associated with this event and I will NOT hold Warwick Valley Humane Society, Greenwood Lake Chamber of Commerce and it’s Winter Carnival Committee, Greenwood Lake Police, Fire and Ambulance, or any sponsors, volunteers and Warwick Valley Humane Society Board of Directors liable for any circumstances related to this event.
I hereby confirm that I am 18 yrs or older and in good physical condition and do not suffer from any disabilities or physical conditions that places me at risk or otherwise should prohibit my participation in this “Go Jump in the Lake” Polar Plunge fundraising event.
Furthermore, in consideration of my participation in this program, I, myself, my heirs and assigns, release the Warwick Valley Humane Society, Greenwood Lake Chamber of Commerce/Winter Carnival Committee, their sponsors and any volunteers, from any and all claims, actions, demands, expenses liabilities (including reasonable attorney’s fees) and negligence made or brought by myself, my heirs or assigns, as a result of participation in this event.
I, for myself, further agree not to sue any of the Officers, agents, affiliates, employees, contractors, or volunteers of Warwick Valley Humane Society, Inc. or Greenwood Lake Chamber of Commerce and it’s Winter Carnival Committee, as a result of any injury, paralysis or death that might be suffered in connection with participation in this event.
Participants are required to wear closed footwear due to safety concerns and the fact that numb feet may not feel objects in the water.
